Nondiscrimination notice
Shin Family Medicine & Laser Center complies with applicable federal civil rights laws and does not discriminate on the basis of race, color, national origin (including limited English proficiency and primary language), sex, age, or disability.
We provide, free of charge and in a timely manner:
- Reasonable modifications and auxiliary aids and services for people with disabilities, such as qualified sign language interpreters and information in other formats (large print, accessible electronic formats).
- Language assistance services, such as qualified interpreters and translated documents, for people whose primary language is not English.

How to file a civil rights complaint
If you believe we have failed to provide these services or discriminated in another way, please tell us so we can make it right. You can also file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, electronically through the OCR Complaint Portal, or by mail at: U.S. Department of Health and Human Services, 200 Independence Avenue SW, Room 509F, HHH Building, Washington, DC 20201.
